When developing the project plan, a project manager is often under pressure to produce a plan which meets the (unrealistic) expectations of some of the stakeholders. It is important here that the project manager comes up with a realistic schedule - one which he/she thinks is realistic to achieve. What you put in the Charter is essentially the objective of the project, the scope of the project, what things will be done to complete the project, and who it is that needs to formally authorize the project. And, the best part of the Charter is that if it isn't in the Charter, you can't do it. This keeps your boss and others from continuously changing their minds and not letting you finish what you started.
The implementation phase is about tracking and managing the project. The first thing that happens when the project begins is to use the Project Definition Report to create a project plan which defines how to perform what is detailed on the PD report. The PD is more of a summary of the project, so a detailed project plan must be created to fill in the fine detail of how the project will be run. The project plan is the central document that is used to manage the project for its duration so getting agreement and acceptance from all of the team on aspects such as the project milestones, phases and tasks, as well as who is responsible for each task, associated timelines and what deadlines are to be met.
Other than the Charter, lessons learned is probably the other most neglected part of a project. It is reasonable to think that since projects are new there will be unanticipated obstacles that you run into.
